ApoB: the number that predicts heart risk

ApoB counts the particles that drive heart disease. It can tell you more about your risk than cholesterol alone.

What is ApoB?

ApoB is a protein that sits on cholesterol carrying particles. Every atherogenic particle, namely LDL, IDL, VLDL remnants and Lp(a), that can lodge in an artery wall carries exactly one of them.

So counting ApoB counts those particles directly. It measures how many there are, not how much cholesterol each one holds inside.

In practice, measuring ApoB in addition to the standard lipid panel helps us to better understand your cardiovascular risk.

Two people can carry an identical cholesterol level. If one packs it into many small particles, that person usually runs the higher risk.

When ApoB and LDL disagree

LDL cholesterol and ApoB usually move together, so knowing one often tells you the other. Occasionally they do not, and that is when ApoB is most useful.

Your LDL can read normal while your ApoB runs high. This happens when your particles are small and numerous, so each carries a little less cholesterol.

This matters more in Singapore. The pattern is common in insulin resistance and metabolic syndrome, both widespread here. It is exactly the person a standard cholesterol panel can wave through.

LDL measures the cargo. ApoB counts the vehicles.

Ranges come from the PROTOCOL biomarker reference. Your own target depends on your wider heart risk, so agree one with your doctor.

What raises it

Diets high in refined carbohydrate and saturated fat can push it up. So can excess weight, smoking and a sedentary routine.

Genetics play a real part too. Some people run high despite eating well and staying active, through no fault of their own.

A few conditions raise it as well, including an underactive thyroid. Your doctor will look for these if your number is high.

What lowers it

The habits that help your cholesterol help here too. A better diet, more activity, a healthier weight and not smoking all lower the particle count.

ApoB is also the number most cholesterol treatments are built to bring down. If yours stays high despite good habits, that is a conversation with your doctor.

Should you check it?

It is worth knowing if heart disease runs in your family. It is also useful when your LDL looks fine but your risk still feels unclear.

If you have already had a heart attack or stroke, it helps guide treatment. It shows whether your particle numbers are low enough for your risk.

How it is measured

ApoB is a simple blood test, usually added to a lipid panel. Most modern labs do not require you to fast beforehand.

It is widely available and not expensive. The main barrier is simply that it is not ordered by default.

The bottom line

ApoB counts the particles that actually cause the harm. It often catches risk that LDL alone can miss.

If you add one heart number this year, this is a strong candidate. Read it with your doctor, alongside your other results.
